E-911 Address Request Form

Clearwater County requires two things to be in place before we can assign an E911 Physical Address.

1.  Driveway that will be used to access your house must be installed/completed.

2.  Building must be present or new construction must be started and visible, Clearwater County does not assign addresses to empty parcels of land.
